How to File a Grievance

Our goal at Destiny Home Health is to provide excellent patient care and healthcare services to our local communities.

If you believe that Destiny Home Health Agency has failed to provide these services or discriminated in any other way, you may file a grievance with our Civil Rights Coordinator by phone at (951) 242-1365, by email at destinyhomehealth@hotmail.com or in person or by mail at 12370 Perris Boulevard, Moreno Valley, CA 92557. If you need help filing a grievance, our Civil Rights Coordinator is available to help you

The availability and use of this grievance procedure do not prevent you from pursuing other legal or administrative remedies.

You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights by any of the following methods:
