Who is Destiny Home Health Agency?

Dedicated to serving three counties, Riverside, San Bernardino, and Los Angeles, we employ dedicated professionals who place patients first and treat them as they would treat their families. Destiny Home Health Agency was founded by Joy Ekpo in 2004. She personally assembled a caring, compassionate, skilled, and personal care team to implement her vision. Destiny Home Health Agency employs a diverse team of multi-cultural and multi-lingual professionals to meet the needs of the population it serves.

COVID-19/ CORONA VIRUS GUIDELINES FOR OUR PATIENTS, FAMILIES AND THE COMMUNITIES WE SERVE:

ALL DESTINY SERVICES WILL REMAIN AVAILABLE TO OUR PATIENTS, THEIR FAMILIES, AND THE COMMUNITIES WE SERVE DURING THIS PANDEMIC

Destiny stands with our patients, families, and communities of over 8,500 beneficiaries currently receiving home health, hospice, and palliative care services in their private homes, Assisted Living Facilities, Board & Care, and Skilled Nursing Facilities.

In order to protect our vulnerable patients, we have implemented the following safety precaution guidelines to minimize any unnecessary exposure to potential infection caused by the virus.

PRECAUTIONS DESTINY HAS IMPLEMENTED ACROSS ALL OUR AGENCIES:

  • Employees are required to start from home rather than from the office. They must also check their temperatures daily.
  • Employees are not allowed to report to work or offices if they are feeling sick or experiencing any symptoms.
  • Employees are pre-screening all our patients and their families for symptoms before making a home visit. If any patient or family member reports any symptoms, our staff will not make a home visit, the PCP will be notified instead. The patient will then be instructed to go to the nearest ER or hospital.
  • Employees are directed to use all infection control measures while providing care to our patients including, but not limited to: hand sanitizers, hand washing, Sani-Wipes to clean equipment, and more.
  • Patient visits for Hospice and Palliative services have been reduced to 1-2 weeks and as needed. These services are however, available telephonically 24/7. Nursing and Aide services will be maintained. Other team visits have been reduced to an as needed basis or put on hold at this time.

PRECAUTIONS DESTINY IS ADVISING OUR PATIENTS AND FAMILIES TO FOLLOW:

  • Social Distancing: Stay at home, maintain 6 feet of distance from people, and limit visits from younger individuals who may be asymptomatic and can spread the virus. (Etc. grand kids, in-laws, and others).
  • Medication(s): Continue to take your medication(s) as ordered by your doctor and educated by your nurses.
  • Diet and Hygiene: Eat healthy and well-balanced meals. Cover your coughs and sneezes. Immediately report any fever, shortness of breath, or other symptoms/illness to your doctor and nurses.
  • Call First: Please refrain from going directly to the ER, Urgent Care, and your doctors’ offices. Instead, call Destiny for a phone or video consultation first.
  • Safety: You are safer having one nurse come to your home than sharing the waiting room with 20 or more individuals who may have symptoms of the coronavirus.

Together, we must free up emergency rooms for people who need acute care and limit exposure to the virus.

DESTINY HAS DOCTORS AND NURSE PRACTITIONERS WHO ARE AVAILABLE FOR VIDEO CONSULTATIONS, USE OF TELEHEALTH EQUIPMENT, AND MORE TO CONTINUE CARE FOR OUR PATIENTS AT THEIR PLACE OF RESIDENCE DURING THIS DIFFICULT TIME.
