Our community liaisons are an integral bridge in the home health care process. Our staff takes pride in building strong relationships with local doctors, hospitals, referral coordinators, and other providers, as well as educating our local community about our Home Health, Hospice, and Palliative care services.

Community liaisons assist with the transition and education of our patients from the hospital, doctor’s office, or clinic back to their homes.

Destiny’s community liaisons are ever ready to guide and support our patients through the referral and intake process to make the transition to home care as coordinated and smooth as possible; they are eagerly available for questions, concerns, and emotional support.
